Check with your sponsor to determine exactly which of their membership centers will sponsor your position.
If your sponsor has instructed you, list your sponsor's name as "John Smith-1" or "John Smith-2" where the number represents John's first or second position he bought. This is how you indicate which position is sponsoring you. Your sponsor could have many positions. If you were not given this information, simply list your sponsor's name.  Provide any special instructions in the COMMENTS section.
